We can workthrough this crisis-together."BAYADA Home Health Care is currently seeking an experienced Licensed PracticalNurse, LPN, for a FULL-TIME position performing home health visits for ourFort Collins Visits office. This office services adult and geriatric clientson a per visit basis in territories throughout Larimer and Weld counties.As a home care nurse, you will be an integral member of a multi-disciplinaryhealth care team that provides skilled nursing and rehabilitative care toclients, affording them the opportunity to receive the medical care requiredto remain at home.Responsibilities for Licensed Practical Nurse (LPN):Making home visits to clients living in designated territories.Performing assigned duties, including administration of medication, woundcare, treatments, and procedures.Monitoring clients' conditions; reporting changes to Clinical Manager andClient Services Manager.Following up with, executing, and properly documenting doctors' orders.Performing client assessments as necessary.Case management and coordination.Accurately document observations, interventions, and evaluationspertaining to client care management and services provided, utilizingelectronic medical records on a state-of-the-art touch pad tablet.
