Our House Inc. Direct Care Support in Fort Collins, Colorado

Position Summary

It is the goal of Our House Inc to provide an environment focused on individualized programming that allows individuals in services an opportunity to continue learning in order to reach their full potential academically, vocationally, mentally, socially, and emotionally.

The Skills and Activity Provider is responsible for the overall daily activities, welfare, and progress of the individuals in their care at Our House Inc. The Skills and Activity Provider will often be the first point of contact for individuals in services and families with Our House Inc., and needs to set a standard of goodwill and service with those they meet. It is in the S&A Provider description to implement daily activities, goals, and learning experiences to help enrich, teach and grow the individuals that are in their care.

Planned daily activities can include but are not limited to: volunteer work, vocational training, exercise, arts and crafts, games that encourage critical thinking, and strategies that teach life skills such as cooking, cleaning, self-control, hygiene, etc. Providers are encouraged to create individualized programming based on individuals? personal interests and needs, as well as identified goals.


  • Ensure the work, attitude, conversations, and goals directly support the mission statement of Our House Inc.

  • In all interactions with individuals receiving services, parents, coworkers, school Providers, and agency representatives are a positive embodiment of Our House Inc.

  • Maintain a clean and presentable work environment by picking up after self and individuals coming to services; and also taking initiative to address areas within the Agency?s center that can be improved.

  • Maintain a neat and presentable appearance; business casual.

  • Promote community awareness of the abilities of the individuals with disabilities while maintaining reasonable confidentiality.

  • Attend and participate in all required trainings and meetings that will promote increased ability to work with students and/or understand your role as an S&A Provider.

  • Facilitate or provide transportation to planned daily activities as well as to and from the Agency, as needed.

  • Will actively engage and provide support for students to fully participate in daily activities.

  • Respect and safeguard the rights of the students and maintain appropriate supervision.

  • Implement and provide data tracking for plans such as Behavior Support Plans (BISSP) when appropriate.

  • Be willing and able to assist with Activities of Daily Living (ADL?s) including but not limited to: cleaning face, brushing hair and teeth, assisting with clothes, feeding, toileting, changing, showering, etc., when necessary.

  • Be a positive point of contact and information to students? Providers on a regular basis.

  • Participate in the implementation of the Individual Service and Support Plan (ISSP) within the student?s daily activities.

  • Attend and participate in any Individualized Plan (IP) or Individualized Education Plan (IEP) for students as needed.

  • Document and report incidents appropriately with the given time frame.

  • Document the daily service provided and activities participated in (Daily Activity Reports)

  • Have all paperwork completed to Our House Inc. standards and submitted in a timely manner.

  • Other duties as needed.


  • Must be willing to take a criminal, MVR, and CAPS background check. (Any offense may not necessarily disqualify you from working for Our House Inc.)

  • Must have valid driver?s license.

  • Must have vehicle insurance/ registration, and a good driving record if transporting.

  • Must be 18 years of age or older.

  • Must have current CPR and First-Aid certifications.

  • Must take drug screen and pass.

  • Must be able to lift a minimum of 50lbs; if required.

  • Must be in good physical, mental and emotional health.

  • Must be dependable, reliable, and punctual.


  • Vehicle preferred, must be in good working order if transporting.

  • Must be able to demonstrate the ability to work independently.

  • Experience working with developmentally disabled children and/or adults preferred.

  • Experience working with seizure disorder and behaviors preferred.

  • Associate?s or Bachelor?s degree within the human service field or equivalent experience desired.

  • Experience working or advocating for individuals with disabilities preferred.

  • Must have good critical thinking and problem solving skills.

  • Must have good interpersonal and communication skills.
