General SummaryThis position provides patient care for adult patients in the outpatient SleepMedicine clinic setting. Practice will focus predominantly on management offollow-up patients with sleep-disorded breathing and will include ongoingmanagement of CPAP and other DME. Serves as clinical coordinator for clinicalresearch as needed. This includes initial screening of patients, monitoringof study patients throughout their participation in studies and helping tomake clinical recommendations for medical management after completion ofstudy.Essential Responsibilities1. Provide care for adult patients in the Department of Medicine clinicalsleep medicine program.2. Performs follow-up visits gathering data from initial evaluation,develops and documents an assessment and plan of care.3. Monitors progress of each patient and revises orders and care plans asneeded. Identifies problems that may need special attention or more acutecare.4. Supervises ongoing care including use of durable medical equipment tofacilitate long-term management of sleep disordered breathing. Consults withancillary services and specialties as necessary.5. Generates progress notes with each outpatient visit. Keeps problem sheetsup to date. Maintains communication with referring physicians and otherhealth care providers.6. Assists medical team in determining a home care plan for patient andprovides appropriate care. Mobilizes appropriate persons to contact resourcesfor continuation of recommended treatment program.7. Serves as clinical coordinator for research protocols, which includesrecruiting and screening of patients, performing necessary testing andmeeting with study monitors.8. Performs all other duties as assigned.General Responsibilities1. Performs, as directed, safety compliance and uses Personal ProtectiveEquipment (PPE), as needed.2. Participates in Quality Assessment (QA) and Quality Improvement(QI) programs, as directed.3. Ensures compliance with The Joint Commission and all other Federal, Stateand Regulatory Agencies.4. Responds promptly and sincerely to customer's needs, requests andconcerns via all communication forms using easily understood language andrefraining from using inappropriate language and non-verbal gestures.5. Speaks positively about the institution,provides customers with prompt service, maintains a clean and safe workingenvironment, dresses appropriately based upon National Jewish safetystandards, and departmental policies and wears an ID badge visibly.Knowledge and SkillsNoneEducationBS required in Child Health Associate Program or other approved PhysicianAssistant Program. MS or equivalent work experience preferred.Certification and LicensurePhysician Assistant Certificate required. Colorado State Board of MedicalExaminers License required. Healthcare provider level CPR/BLS requiredevery 2 years. Staff assigned to high-risk areas (Cardiology, MIDC,PCU, Infusion Room) must maintain ACLS or PALS as appropriate.
