Denver Health Pharmacist Clinical Specialist - Pediatrics in Denver, Colorado

Job Summary Under minimal supervision, provides medication therapy management services and collaborates with other health care providers to optimize therapeutic outcomes for Denver Health patients. Documents all patient care activities in the patient medical record. Develops policies and procedures, clinical guidelines, or other documents required to operate the service. Plays active role in pharmacy department quality initiatives and staff education.

Minimum Qualifications Education: Bachelor's Degree in Pharmacy required, PharmD preferred.

Experience: At least one year of experience in the provision of medication therapy management services to patients. Completion of a PGY 1 Pharmacy Practice residency or equivalent experience is preferred. Completion of a PGY 2 Specialty residency is preferred. Board certification in Pharmacotherapy (BCPS) or other specialty as needed is preferred.

Knowledge, Skills & Abilities: Broad pharmacotherapy knowledge for managing patient medication therapy needs, including current treatment guidelines and literature. Strong communication and active listening skills. Ability to provide input into the vision and development of clinical pharmacy services.

Certificate/License/Registration: Licensed by the State of Colorado Board of Pharmacy Registration at the time of hire. All Certifications and Licenses required for this job must be kept current as a condition of continued employment.
