Metro Community Provider Network (MCPN) is a 501(c) (3) Federally Qualified Health Center (FQHC) with a history of providing primary care and other health-related services in a community-based setting. Established in 1989 in response to the primary health care needs of the medically indigent population in suburban Denver, MCPN has the largest population base of any FQHC in Colorado and provides primary care, dental, pharmacy, behavioral health, health education and outreach and enrollment services in accordance with its mission.

The Refugee Health & Community Care Manager RN (aka Refugee Nurse) is a practice-based RN who directly supports MCPN’s Refugee Services line, as well as the Refugee Medical Screening program. The Refugee RN will also serve as a team member to MCPN’s Nurse line, and contribute to integrated care delivery. In collaboration with other health care team members, the position is responsible for organizing, coordinating, and providing screening support per the mandated/contractual guidelines, care coordination and case management services to Refugee patients within MCPN’s system who are most at risk for health deterioration, sentinel events, and/or poor outcomes.
Refugee Screening Program & Health Promotion
Oversee Refugee health screening program in partnership with medical providers, behavioral health providers, operational staff, etc., and coordinate the orientation and transition to medical homes for newly arriving refugee patients.
Develop, implement, update, and document programmatic protocols and procedures.
Assure compliance with state and federal refugee screening guidelines, grant and contract requirements, including reporting out.
Develop and monitor systems for scheduling physical and behavioral health follow-up, along with other supportive services for newly arriving refugee populations seen by MCPN.
Manage Refugee Screening Contract and Refugee Health Promotion Grant deliverables.
Work collaboratively and coordinate service delivery across MCPN departments, within the Elmira Refugee Clinic, and with partner organizations to support clinic and program operations.
Care Management Systems:
Manage MCPN’s higher-risk refugee patients, as determined by MCPN’s risk stratification system.
Partner with MCPN Complex Care Coordination and actively participate in MCPN’s systems for identifying high risk and most complex Refugee patients through Electronic Health Record (EHR), referrals, registries from outside agencies and community partners.
Direct Patient Care:
Provide health related counseling on the prevention of communicable diseases, utilization of healthcare systems, introduce general health related information and instruction to refugee individuals, families and community groups.
Keep accurate patient records; maintain patient confidentiality at all times.
Compile and submit necessary medical/statistical reports and data to internal departments or external partners in a timely fashion and, if applicable, as required by law.
Conduct comprehensive assessment of patients’ physical, mental, and psychosocial needs.
Administer a variety of pre-examination tests including skin tests, blood tests, and other tests used in determining communicable disease; provide information and education to patients before and after tests as necessary.
Participates in the delivery of clinical care within scope of nursing and as a team member of MCPN’s healthcare delivery team. This might include medical triage, authorizing medication refills, and other in-scope services crucial to the delivery of timely and efficient healthcare.
Develop care plans to prevent disease exacerbation, improve outcomes, increase patient engagement in self-care, decrease risk status, and minimize hospital and emergency department utilization.
Utilize behavioral strategies to help patients adopt healthy behaviors and improve self-care in chronic disease management.
Assist patients in navigating the health care system. Coordinate specialty care, follow-up on test results and other care coordination needs.
Collaborates with other departments, external organizations and consultants, including regulatory agencies and coalition partners, to support and coordinate patient care and to maintain MCPN’s regulatory compliance.
Participates in and helps maintain an On-Call program that supports the MCPN “Doc Line”, supports clinical needs of MCPN’s call center, and interfaces with our patient care referral networks and hospitals.
Participates in and helps to maintain a system for first pass review of specific EHR desktop items that are within the scope of an RN.
Provides first line coverage for EHR desktops when providers are on PTO/FMLA/Floating Holiday.
Manages Refugee Health Promotion staff members.
Education or Formal Training: Training from accredited educational institution or school as an RN.
Experience: At least 2 years experience in a clinical setting post-graduation from highest degree. Previous leadership preferred; experience in a large healthcare organization a benefit. Background in public health, case management/care management, human services, or social services preferred. Experience working with refugee and immigrant populations preferred. Bilingual a plus, but not required. MCPN conducts a background check, including criminal history, education, license, certification, and drug screening.
