The College of Nursing has an opening for full-time RN/Bilingual Case Manager position within its federally qualified community health center, Sheridan Health Services (SHS), located in southwest Denver, CO (zip code: 80236).

Nature of Work:

The University Of Colorado College Of Nursing, is seeking a Bilingual RN/Case Manager for a full time, 1.0 FTE, benefitted position at its Sheridan Health Services (SHS) Community Clinic, in Denver, CO (80236). This RN should be team oriented and willing to uphold the Sheridan Health Services (SHS) mission by assuring that patients receive health care that is competent, caring, cost-effective, and met with a high degree of satisfaction. Sheridan Health Services is a Patient-Centered Medical Home, and as such we uphold a model of care that places the patient at the center, incorporating the values and beliefs of the patient in all decision-making. This position is responsible for providing patient care coordination including helping patients connect with specialty care, provide clinical care for children and adults, help families access available community services, as well as triaging patient phone calls throughout the day.

Clinic Description:

Sheridan Health Services is a multi-site, nurse-managed federally qualified community health center, funded through the Health Resources and Services Administration Bureau of Primary Healthcare; and is a collaborative effort between the College of Nursing and the Sheridan School District. There are now 45 employees at two convenient locations to serve community health needs primarily for residents of Arapahoe, Denver and Jefferson counties. A Youth Health Clinic is located in the Sheridan School District Administration building. The Family Health Clinic is located in southwest Denver on the Fort Logan campus and provides services to adults and children, 65% of whom identify at Latinx, and 42% of whom are best served in another language, with Spanish being the predominant second language. Services offered include primary care across the lifespan; prenatal and postpartum, gynecology, dentistry, pharmacy, behavioral health, and outpatient substance abuse treatment. Depending on qualifications and department needs, appointments may be made at the Entry Professional, Professional, Senior Professional or Principal Professional level.

Examples of Work Performed at All Levels:

Care Coordination

  • Telehealth Champion across all service lines: Adult & Pediatric Primary Care, Behavioral Health, Psychiatry, Obstetrics

  • Coordination across service lines for mutual patients (ie Adult & Pediatric Primary Care, Behavioral Health and Psychiatry)

  • Actively searches for beneficial programs and services for SHS patients.

  • Maintains informative brochures and educational materials.

  • Meets with patients to assess social and medical needs.

  • Works with other SHS providers to provide comprehensive and integrated care.

Phone Triage

  • Speaks with patients that call the clinic with health questions and provides guidance.

  • Works with providers to address patient care needs over the phone.

  • Calls patients with testing results and health education.

Health Coaching

  • Encourages patients to set positive lifestyle changing goals in order to improve overall health and well-being.

  • Follows up with established patients on a regular basis and helps them to assess goal progress and recognize achievements.

Student/Medical Assistant support and training

  • Assists with orientation for new students, faculty, and staff to the clinic.

  • Serves as a resource for MAs and support staff to assist with patient care.

Additional Posting Description

  • Focuses on population based health to improve preventive care/immunizations rates and chronic disease outcomes.

  • Ensures infrastructure for implementing, tracking and reporting progress on quality and grant-related activities and measures.

  • Documents patient care using the electronic medical record system.

  • Takes an active role in promoting and building the integrated health team.

  • Participates in community outreach events. (Weekend availability for participation as needed)

  • Remain credentialed ensure credentialing materials stay current.

  • Provides coverage during absence of other RN staff at both clinic sites.

  • Takes on assigned projects as needed.

At the entry level, duties are limited in scope and are performed with guidance and direction from other professionals and are performed in a training and development capacity.

At the professional level, duties may be more limited in scope and are performed with guidance and direction from other professionals.

At the senior professional level, work is performed fully independently. In addition to the duties described above, positions at this level are responsible for:

Care Coordination:

  • Networks with outside specialty providers to establish low/no cost care for patients.

  • Helps connect patients to specialty providers when needed.

Health Coaching:

  • Meets individually and in groups with patients to assess current health and care needs.

  • May work with other staff to develop health promotion

Student/Medical Assistant support and training

  • Assists with orientation for new students, faculty, and staff to the clinic.

  • Serves as a resource for MAs and support staff to assist with patient care.

  • Provides training to students

Additional Posting Description

  • Actively reviews grant deliverables and monitors grant work plans in order to meet deliverables.

At the principal professional level positions are either work leads over others or are considered a subject matter expert in a particular area. In addition to the duties described above, positions at this level are responsible for:

Health Coaching:

  • Leads group education and health promotion initiatives. This may include developing marketing materials, recruiting patients, developing curriculum and leading Group sessions.

  • Leads staff training initiatives as appropriate

Student/Medical Assistant support and training

  • Develops and leads orientation for new students, faculty, and staff to the clinic.

  • Serves as a resource for MAs and support staff to assist with patient care.

  • Heavily involved in student training. This may include development of course activities and projects that are aligned with course curriculum, as well as student oversight and evaluation

  • Supervision and professional development of Medical Assistants

Salary and Benefits:

  • At the entry level the salary range begins at $47,025 and is commensurate with skills and experience, and the position is eligible for overtime compensation.

  • At the professional level the salary range begins at $51,175 and is commensurate with skills and experience, and the position is not eligible for overtime compensation.

  • At the senior professional level the salary range begins at $55,324 and is commensurate with skills and experience, and the position is not eligible for overtime compensation.

  • At the principal professional level the salary range begins at $63,622 and is commensurate with skills and experience, and the position is not eligible for overtime compensation.

Professional:

  • Graduate with Bachelor’s degree in nursing from an accredited school of nursing

  • 1 year of professional level RN experience

  • Current RN licensure in good standing (any state)

  • Could be working toward CO clinical license during application process and registered with DORA-Department of Regulatory Agencies. Must have CO licensure by date of hire.

Substitution:

A combination of education and related technical/paraprofessional experience may be substituted for the bachelor’s degree on a year for year basis, but not for the specific 1 year of experience.

Senior Professional:

  • Graduate with Bachelor’s degree in nursing from an accredited school of nursing

  • 2 years of professional level RN experience

  • Current RN licensure in good standing (any state)

  • Could be working toward CO clinical license during application process and registered with DORA-Department of Regulatory Agencies. Must have CO licensure by date of hire.

Substitution:

A combination of education and related technical/paraprofessional experience may be substituted for the bachelor’s degree on a year for year basis, but not for the specific 1 year of experience.

Principal Professional:

  • Graduate with Bachelor’s degree in nursing from an accredited school of nursing

  • The position requires extensive, diverse and progressively responsible experience as an RN

  • Current RN licensure in good standing (any state)

  • Could be working toward CO clinical license during application process and registered with DORA-Department of Regulatory Agencies. Must have CO licensure by date of hire.

Substitution:

A combination of education and related technical/paraprofessional experience may be substituted for the bachelor’s degree on a year for year basis, but not for the specific experience.

Conditions of Employment:

  • Must be willing and able to utilize bilingual skills (Spanish/English) at a conversation level in a clinical setting.

  • Must be willing and able to obtain Colorado RN license by date of hire.

  • Must be willing and able to obtain current BLS/CPR certification by date of hire.

  • Must have valid nation provider identifier (NPI) number by date of hire.

  • Must be willing and able to remain credentialed and ensure credentialing materials stay current.

  • Must be willing and able to work in normal patient care environment with some exposure to biological hazards and infectious diseases.

  • Must be willing and able to pass a sex offender background check.

Preferred Qualifications:

  • Experience providing care coordination in a Federally Qualified Health Center, a public health setting, and/or to low income families

  • Experience in program development and implementation

  • Case Management certification and/or 2 years or more experience with case management

  • Experience managing personnel

Competencies:

  • Considerable degree of independent judgment and work priorities is necessary to monitor and respond to changes in a client’s condition.

  • Interpersonal skills to work effectively with patients, their families, and other members of the interdisciplinary health care team.

  • Skilled in the use of the electronic medical health record and other clinic software

  • Basic knowledge of Colorado state immunization requirements

  • HIPAA and OSHA compliance

  • Intermediate level computer competency

  • Exemplary organizational skills and ability to prioritize organizational needs
